What does this painting mean to you - if anything at all?

Found this painting on YouTube. I don't know the name of it, though I'd like if someone here could tell me.
To me, it has two different meanings (speaking from personal experiences):
  1. The first meaning is the struggle of dementia or Alzheimer's disease. You slowly begin to lose your grasp on the world, and return to the form of a child but in an adult's body. To me, the face "falling" - so to speak - is representative of a man losing himself without any way of stopping it. I know this struggle personally as my grandfather is dealing with dementia. It's a terrible illness that I would not wish on my worst enemy.
  2. The second meaning is to deal with the loss of your self-esteem, or at the very least the degradation of it. The face falling, once again, represents the loss of confidence in your own being and your own soul. The man in the painting is unable to escape his loss of self-esteem and self-worth, thus resulting in a fractured or desecrated face. This is yet another personal struggle as I have dealt with many blows to my self-esteem throughout the years, thus making me feel like I don't matter.
Anyway, I'm curious on your opinions.
This is the beautiful thing about art, no? We can all form different meanings on a piece.

"ego" death feels like this. my personality vanished, I cried and begged God multiple times for help, this lasted a few days. it was a mistake, but I almost died from an overdose, it made me die mentally, and after it I wished I could truly die
